• Your answers will help inform improvements to service accessibility, relevance and delivery.
• Your answers will be kept confidential.
• It will take less than one minute to answer the questions.
• You do not have to answer the questions if you do not want to.
• You can skip the questions and still use the chatbot. The services you receive from WDG Public Health will not be affected by your decision to answer these questions.
• Results will be used for internal program planning and may be shared with WDG Public Health’s Board of Health. Any information shared will not identify you.
